At Five Bridges, we understand that transitioning back into the community can be challenging. Our residential support program provides a safe, stable, and culturally supportive environment where participants can rebuild their lives, reconnect with their goals, and work towards greater independence.

From the moment a referral is received through to securing long-term housing and employment opportunities, our dedicated team is here every step of the way.

The journey begins before release from custody. Referrals are received through correctional services, prison liaison officers, and partner organisations. Our team carefully reviews each application and works with correctional staff, parole services, and support agencies to understand each person's circumstances and identify the assistance they may need upon release.

Once accepted into the program, participants receive confirmation of their placement and our team begins planning for a smooth transition back into the community.

On the day of release, a Five Bridges Community Service Officer welcomes participants and provides immediate support to help them settle into life outside custody.

This includes assistance with:

  • Transport from the correctional facility
  • Attending Probation and Parole appointments
  • Connecting with Centrelink and other essential services
  • Moving into residential accommodation
  • Understanding house expectations, safety procedures, and available supports

Our goal is to reduce stress and ensure participants have the support they need from day one.

Additional Information

Clients are required to meet the following criteria:

18+ years of age

Petty criminal offences only

The first week focuses on building stability, wellbeing, and connection.

Participants work closely with their Community Service Officer to develop an individual support plan that reflects their goals, strengths, and needs.

Support may include:

  • Obtaining identification documents
  • Accessing healthcare and counselling services
  • Connecting with alcohol and drug support programs
  • Accessing food, clothing, and other essential items
  • Attending appointments with the support of our team

Participants are also encouraged to take part in culturally supportive and life-skills programs that promote personal growth and community connection.

As participants become more established, support focuses on developing confidence, life skills, and independence.

Our team assists with:

  • Employment and job-seeking support
  • Housing applications and accommodation options
  • Medical, wellbeing, and counselling appointments
  • Budgeting and daily living skills
  • Developing transport skills and community access

Participants are encouraged to gradually take greater ownership of their appointments and daily responsibilities while continuing to receive guidance and support when needed.

Throughout their stay, participants continue to engage in activities and programs that support their personal development, wellbeing, and connection to community.

This may include:

  • Cultural and yarning programs
  • Life-skills development activities
  • Health and wellbeing supports
  • Community participation opportunities
  • Ongoing case management and mentoring

Our team works alongside participants to monitor progress, celebrate achievements, and address any challenges that arise.

Support doesn't end when a participant leaves our residence.

We remain connected through ongoing follow-up and wellbeing support to help ensure a successful transition into independent living. Our team continues to check in, provide guidance, and assist participants in accessing services and opportunities as they move forward in their lives.

At Five Bridges, we're committed to helping people build confidence, restore independence, and create positive futures within their communities.

Acceptance of a place in the Residential Program entails a commitment to pay for rent and ensure the property is maintained at all times, and attend appointments and Activities when required. Clients are responsible for their own rooms and must adhere to the house rules as provided on entry.

Accommodation may be refused at any time before or during the period based on previous and/or current conduct if the client:

·         Poses a safety risk to themselves or other clients

·         Poses a safety risk to the property

·         Poses a safety risk to staff.

·         Is not engaged in any activities/programs whilst residing at the residence

·         Breach of any rule in the signed house rules form.
